Left ventricular (LV) thrombus is a common complication of severe LV dysfunction.[1] Therapeutic options for such thrombi include anticoagulant therapy, thrombolysis, or surgical removal of the thrombus.[2] Clinicians consider hydatid cysts in the differential diagnosis of a cystic lesion, where history and laboratory tests could be used to confirm the diagnosis. But overall, until the diagnosis is established, treatment should also be initiated for a probable diagnosis of thrombus.

Cyst-like thrombi are rarely seen on echo. They have been described, but their thrombotic etiology has been proved indirectly by their disappearance with anticoagulation therapy. Our patient received anticoagulation therapy and there was decrease in the size of the LV cyst by about 35% with anticoagulation, suggesting that the apical cystic image lesion was a thrombus and two-dimensional echocardiography was very useful in the management of this case [Figure 1] and [Figure 2].
